| Core Tip |
This case report highlights a diagnostic pitfall: transient pancreatic enzyme elevation due to a cholecystoduodenal fistula with gallstone ileus, mimicking acute pancreatitis at presentation. The enzymes fell by more than 97% within 24 hours and computed tomography showed no pancreatitis; these findings do not exclude a mild concurrent pancreatitis, but they should not obscure the imaging evidence of mechanical obstruction. Heightened awareness, combined with a rigorous differential diagnostic approach, may prevent delays in definitive surgical management. |
