Rifaximin; Cirrhosis; Mortality; Spontaneous bacterial peritonitis; Hepatorenal syndrome; Bacterial infection; Antimicrobial resistance |
| Core Tip |
Rifaximin is guideline-endorsed for preventing recurrent hepatic encephalopathy. Its effect on other outcomes is uncertain because trials differ in indication, comparator, masking and follow-up. The updated mortality pool was significant with conventional inference, but not after small-sample adjustment. Blinded and open-label strata showed no clear difference. Low- and very-low-certainty spontaneous bacterial peritonitis and hepatorenal syndrome signals are hypothesis-generating. Guideline-supported use for recurrent hepatic encephalopathy is unaffected; evidence is insufficient to extend rifaximin routinely to other outcomes. |
