Core Tip Levomepromazine-associated priapism is rarely reported despite the drug’s substantial α1-adrenergic antagonistic activity. This case is notable for the development of severe ischemic priapism only 48 hours after initiation of levomepromazine monotherapy at a therapeutic dose, with complete absence of penile pain despite profound cavernosal hypoxia and acidosis. Failure of intracavernosal phenylephrine ultimately required surgical shunting. This observation emphasizes that absence of pain does not exclude severe ischemia and that persistent erections during antipsychotic therapy require prompt evaluation and treatment to prevent irreversible cavernosal injury.
