Core Tip Among 42 women aged ≥ 80 years with high-grade serous ovarian cancer, most had substantial comorbidity and advanced-stage disease. Primary cytoreductive surgery (PCS) was performed in 66.7%, with complete macroscopic cytoreduction achieved in 64.3%. Despite postoperative complications in 35.7%, median progression-free and overall survival were 25 and 37 months, respectively. PCS and better Eastern Cooperative Oncology Group performance status were independently associated with improved survival. These findings suggest that carefully selected octogenarians can achieve meaningful outcomes and that treatment decisions should prioritize functional status and comorbidity burden over chronological age alone.
