Core Tip	Amelanotic cellular blue nevus of the iris is an exceptionally rare benign melanocytic lesion that can closely mimic amelanotic uveal melanoma. We report a 56-year-old Asian Indian man with recurrent hyphema and a vascular iris mass whose magnetic resonance imaging favored melanoma, while anterior chamber cytology was inconclusive. Sectoral iridectomy with histopathology and immunohistochemistry established the diagnosis of amelanotic cellular blue nevus. This case highlights the limitations of imaging alone in differentiating benign from malignant iris tumors and emphasizes tissue diagnosis to avoid unnecessarily aggressive treatment.
