Core Tip This review reframes metabolic dysfunction associated steatotic liver disease as a cardiovascular and metabolic risk signal, not merely a liver disease. The core framework is as follows: Use the fibrosis-4 index for first-layer fibrosis risk triage; identify three main clinical phenotypes, including obesity/metabolic burden, atherosclerotic cardiovascular disease, and heart failure/chronic kidney disease; and then prioritize incretin-based therapies or sodium-glucose cotransporter 2 inhibitors. Fibrosis stage serves as an independent risk modifier to guide hepatology collaboration and discussion of combination therapy. Combination therapy remains a second-layer strategy with limited evidence and requires individualized discussion.
