Core Tip Malignant proliferating trichilemmal tumor (MPTT) is a rare cutaneous adnexal neoplasm often misdiagnosed clinically as a benign cyst, as occurred in this case. We report a 58-year-old woman with a rapidly enlarging scalp mass, histologically classified as Group 3 (high-grade) MPTT according to the Ye et al criteria, managed with wide surgical re-excision and adjuvant radiotherapy (50 Gy in 12 fractions), with no recurrence at 6 months after the end of radiotherapy. This case shows that a long-standing, stable scalp cyst that enlarges rapidly should be excised intact and examined histologically, illustrates that a Group 3 tumor can be diagnosed even when invasion is limited and the specimen is fragmented, and, given the short follow-up, underscores the need for long-term surveillance.
