Core Tip The long-term outcomes of endoscopic submucsoal dissection (ESD) in undifferrented (UD) type early gastric cancer (EGC) over 2 cm confined to the mucosa are not well known. In our study, if R0 resection is achieved after ESD, UD type EGCs of over 2 cm showed very good progress over 5 years. Additional 20.4% of patients were able to avoid surgery comparing the treatment methods of expand indication and R0 resection. ESD can be attempted cautiously on UD type EGCs over 2 cm confinded to the mucosa. And additional researchs are needed to determine which UD type EGCs over 2 cm are effective for endoscopic trreatment.
