"Dear Editor,
We appreciate the opportunity to respond to the Track Article Quality Report concerning our published case report. We value post‑publication scrutiny; however, several points in the report require clarification and factual correction.
1) Language and grammar. Before acceptance, the manuscript underwent multiple peer‑review rounds and, per editorial guidance, received certified professional English‑language editing. The edited version was verified and accepted by the editorial office prior to publication. Broad grammar‑centric criticisms post‑publication therefore do not reflect the documented editorial path. If preferred for house style, we are fully cooperative with copyediting adjustments (e.g., interrogative vs. declarative title).
2) “Common topic” characterization. The statement that this is a “common topic at current” is inaccurate. To our knowledge, there are no prior reports that integrate, in the same patient with psoriatic arthritis (PsA) and ulcerative colitis (UC), high‑frequency (18 MHz) musculoskeletal ultrasound for dactylitis together with intestinal ultrasound (IUS), with longitudinal assessment of response to anti‑TNF therapy. Accordingly, the case fills a clear gap by demonstrating an integrated joint–gut ultrasound approach with treatment follow‑up.
3) Ultrasound terminology (use of “Doppler”)
We deliberately retain the term “Doppler”—rather than “color Doppler”—because the parameter of interest in this case is the intensity of inflammatory activity (mural neovascularization), not flow direction. Either color or power modalities can semi-quantify this intensity when using the same standardized settings (low PRF, appropriate wall filter, compatible frequency, controlled gain), ensuring reproducibility across examinations and operators. For style harmonization, we propose wording refinements without changing the term “Doppler”:
• “procedure” → “examination”;
• “high Doppler signal” → “increased Doppler signal (Limberg grade 3)”;
• “Doppler degree” → “mural Doppler grade (Limberg 0–3)”.
To preclude ambiguity, we suggest adding to the Methods:
“In this manuscript, the term ‘Doppler’ is used generically to denote the semi-quantitative mural vascular signal employed for inflammatory activity assessment, irrespective of flow direction.”
4) Probe description and rectal visibility. We used an 18‑MHz linear transducer for dactylitis and a 3.5‑MHz convex transducer for bowel assessment (e.g., sigmoid colon and proximal segments). We did not claim direct rectal mucosal visualization by transabdominal US; rectal involvement was evaluated by colonoscopy. US parameters (e.g., wall thickness and Limberg grade) were used for non‑invasive monitoring.
5) Core Tip concision. We welcome concision. A succinct alternative is:
High‑frequency musculoskeletal ultrasound documents dactylitis while intestinal ultrasound monitors colonic inflammation in PsA with UC. In this case, integrated joint–gut ultrasound tracking paralleled clinical, laboratory, and endoscopic improvement after anti‑TNF therapy.
6) Section placement. The sentence noted under “Chief complaints” can be moved to Rationale/Introduction if preferred. This is a stylistic relocation, not a scientific change.
7) Abbreviation consistency (US vs UC). We acknowledge a typographical oversight where “US” (ultrasound) appears once where “UC” was intended, and we appreciate the opportunity to correct it.
8) Case‑report format and mention of two screened patients. The report presents one patient in detail. Mentioning two initially screened patients—with only one completing protocolized follow‑up—reflects transparent reporting. To eliminate any confusion, we propose the sentence: “Two patients were screened; only one completed protocolized follow‑up and was included in this case report.”
9) Novelty and field impact (with OMERACT GLOUDAS co‑authorship). Ultrasonographic assessment of dactylitis has been standardized— including the preliminary Global OMERACT Ultrasound Dactylitis Score (GLOUDAS), of which I am a co‑author—yet existing publications address joint and intestinal ultrasound separately. Contemporary IUS literature demonstrates utility for monitoring UC, but without concurrent high‑frequency dactylitis ultrasound in the same case. Our report therefore provides a rationale and template for future multicenter studies integrating joint and intestinal ultrasound endpoints with therapeutic monitoring.
In summary, the comments do not compromise the scientific content or originality of the case. We are supportive of minor style harmonization while reaffirming the novelty and clinical value of an integrated joint–gut ultrasound approach with follow‑up in PsA with UC.
