Communication from the editorial team was clear and professional, and the process itself was fair and transparent. That said, there are a few points that could improve the overall experience for authors. One concern is the repeated request for language editing certificates during the revision process. In my view, language editing should ideally be requested only once, at the final stage before acceptance. Asking for it multiple times increases costs for authors and leads to repeated revisions that do not change the scientific content of the work. Another suggestion is to consider increasing the publication frequency to a monthly schedule. More frequent publication would help reduce delays between acceptance and publication and allow faster dissemination of research findings. This is particularly important for clinical and translational studies, where timely publication can have practical implications. Overall, the peer-review process is of good quality and the journal maintains strong academic standards. With small adjustments—especially regarding language editing requirements and publication frequency—the process could become even more efficient and author-friendly. We fully agree with the reviewer that the biological heterogeneity of rectal NETs—particularly differences between margin status and lymphovascular invasion—deserves more granular investigation. As noted, limitations of the current evidence base, including inconsistent reporting of margin status, LVI, Ki-67 index, tumor size, and salvage modalities, precluded more refined subgroup analyses in this meta-analysis. Nonetheless, we agree that future prospective, well-designed studies should stratify outcomes by positive versus indeterminate margins, separate margin involvement from LVI, incorporate Ki-67–based grading and tumor size, and analyze outcomes according to specific salvage strategies. We believe these directions will substantially enhance risk stratification and support individualized decision-making in clinical practice.
