Core Tip Manipulation under anesthesia was an effective treatment for refractory stiff shoulder, but some severe complications had been reported. We presented a rare case of rotator cuff tear and antero-inferior glenoid rim fracture combined with shoulder instability after manipulation for shoulder stiffness. Arthroscopic repair of the glenoid fracture and concomitant rotator cuff tear was performed. A good outcome was achieved with this treatment in the 2-year postoperative follow-ups.
