Core Tip The development of acute kidney injury (AKI) changes the trajectory of the natural history of patients with decompensated cirrhosis or acute-on-chronic-liver failure. With the discovery of newer biomarkers and a better understanding of hemodynamics in patients with AKI and cirrhosis, there is a paradigm shift in the management of such patients. This comprehensive review will focus on the current perspective of the pathophysiology, diagnosis, and management of patients with AKI in cirrhosis.
