This retrospective cohort study evaluated surgical and renal functional outcomes of robot-assisted partial nephrectomy for hilar and non-hilar renal tumors using trifecta and pentafecta metrics. While trifecta reflects perioperative quality, pentafecta incorporates renal functional preservation and chronic kidney disease progression, which are clinically relevant from a nephrological perspective. Using propensity score matching and multivariate analysis, we demonstrate that robot-assisted partial nephrectomy can achieve comparable pentafecta outcomes in hilar and non-hilar tumors when tumor complexity and surgical experience are appropriately considered, supporting the feasibility of nephron-sparing surgery for anatomically complex hilar tumors. |
