This is a retrospective study to assess the impact of thrombocytopenia at presentation and that of platelet transfusion in the management of acute variceal bleeding in patients with chronic liver disease. Ten percent of patients received platelet transfusions and were found to have significantly higher rebleed rates on day 5 and 42 after the index bleeding episode but did not result in significantly higher mortality rates in these patients. On multivariate analysis, platelet transfusion was an independent risk factor for 42-d rebleeding, while hepatic encephalopathy was a significant risk factor for 42-d mortality. |
