Mean platelet volume (MPV), plateletcrit (PLC), and platelet distribution width (PLDW) have been touched on in the literature as parameters of glucoregulation. However, their clinical implications in real-world diabetes care are blurry and unclear. Platelet indices (PIs) if estimated within two hours of blood collection could be useful indicators of glycemic control in type 1 and type 2 diabetes in low-income countries, and reflect the glycemic status in the previous two weeks (the platelet lifespan). Importantly, PIs are not specific and are affected by demographic factors, renal failure, pregnancy, malignancies, and infection. Therefore, combination with blood glucose measurement and the development of local cut-off levels are crucial. PIs have the advantage of reflecting the previous two weeks' glycemic control compared to the HbA1c which reflects glycemic control during the red blood cells' lifespan (120 days). Because of the above, PIs could be useful before surgery, and during pregnancy (with pregnancy cut-off). MPV, and PLC, are more affected by confounders, and because of that PLDW is more specific. |
