Paediatric patients with serious chronic diseases are now often surviving to adulthood and most will need to transition to adult oriented care facilities. This can be a challenging experience for patients, carers and clinicians. For patients who may require admission to an intensive care unit, better engagement with critical care specialists may improve the process of transition, leading to a better patient experience and improved clinical outcomes. |
