Transitioning adolescents living with human immunodeficiency virus (HIV) to adult-centered care in Uganda remains a challenge and critical step for Adolescents’ healthcare, with low transition rates. Thus, has innovative implications: (1) Integrating transition care into existing clinical settings; and (2) Strengthening family support could improve transition outcomes. And, targeted interventions addressing identified predictors can enhance successful transition to adult HIV care. These findings can inform policy makers and practitioners in similar settings. |
