The current surviving sepsis guidelines 2020 recommend fluid boluses of 10-20 ml/kg/bolus, up to 40-60 ml/kg, but recent studies show better outcomes when fluid boluses are restricted. The FEAST trial compared a fluid bolus in children septic shock and reported a 35% higher mortality. Studies have also compared restricted fluid boluses with liberal fluid boluses, with better outcomes observed in the restricted bolus group. This study is an open-level study in children aged 1 month to 14 years, hospitalised for septic shock. Randomization was done to, Intervention Group I (early Inotropes without giving fluid bolus) or comparator, Fluid resususitation Group F, with an allocation ratio of 1:1. There was no statistically significant difference in the initial inotropic support (VIS score), the total duration of ventilation, total length of hospital stay, mortality at 48 hours, or overall mortality between the two groups. In patients with hypotensive shock, mortality at 48 hours was significantly higher in patients receiving fluid boluses. A striking 65% of the patients receiving fluid boluses developed some signs of fluid overload. |
