Core Tip In the pathophysiology of painful diabetic neuropathy, N-methyl-D-aspartate receptor-2B plays a pivotal role. This minireview highlights how it contributes to oxidative stress, neuroinflammation, and central sensitization. It discusses the experimental and clinical data linking N-methyl-D-aspartate receptor-2B upregulation with chronic pain, evaluates emerging therapeutics targeting this receptor, and outlines translational challenges. Understanding the receptor’s molecular interplay with glial cells and signaling pathways provides valuable insight for developing multi-targeted or precision-based therapies to improve painful diabetic neuropathy management.
