This multicenter retrospective cohort study identifies key prognostic factors in cytology-positive gastric cancer patients undergoing gastrectomy. Diffuse-type histology and absence of postoperative chemotherapy were independent poor prognostic indicators. Critically, combination chemotherapy with fluoropyrimidine plus docetaxel or oxaliplatin significantly improved survival, with a 5-year survival rate of 82.4% vs 21.8% for S-1 or cisplatin-based regimens. The chemotherapy regimen was an independent predictor of overall survival. These findings highlight the importance of intensive postoperative chemotherapy in improving outcomes for this high-risk population. |
