CKD after liver transplantation is a modifiable determinant of survival. Beyond CNI toxicity, pre-transplant renal dysfunction, perioperative AKI, and metabolic comorbidities drive risk. Early nephroprotective strategies and tailored immunosuppression are essential to improve outcomes. Proactive perioperative management, delaying CNI exposure, mycophenolate-based CNI minimization, and aggressive metabolic control, significantly improves long-term renal preservation. |
