Core Tip This multicenter retrospective cohort study analyzed 300 patients with acute cerebral infarction to clarify age-related differences in post-stroke depression onset patterns and treatment response. Depression severity and recovery trajectories varied markedly among young, middle-aged, and elderly groups. Younger patients exhibited early-onset but faster remission, whereas elderly patients showed delayed aggravation and poorer response. Age, neurological impairment, inflammatory markers, and cognitive function were independent predictors. These findings highlight the need for age-stratified screening and individualized post-stroke depression management strategies to optimize rehabilitation outcomes.
