Earlier histologic studies from the interferon-based treatment era suggested that HIV coinfection worsened hepatitis C virus (HCV) outcomes. In comparison, recent evidence from Park et al in the DAA era indicates that achieving sustained virologic response abolishes survival differences between individuals living with HIV/HCV and those with HCV monoinfection. Residual adverse outcomes now arise primarily from attrition across the care cascade and from metabolic and non–liver-related risks after cure, rather than from ongoing viral effects. Accordingly, implementing integrated care models within HIV clinics, coupled with long-term metabolic surveillance, is essential to translate virologic cure into durable survival benefits. |
