Preoperative portal venous phase computed tomography (CT) provides quantitative perfusion indicators relevant to anastomotic healing. Decreased normalized portal vein (PV) CT value and reduced PV-to-aorta CT value ratio were significantly associated with postoperative anastomotic stricture after D2 gastrectomy. These parameters, along with body mass index ≥ 25, diabetes, total gastrectomy, and use of a 25-mm stapler, were independent risk factors. Normalized PV CT value showed superior predictive performance among single markers, and the combined clinical-imaging model achieved excellent discrimination (area under the curve = 0.893) and a high negative predictive value (97.9%). These CT parameters enable preoperative risk stratification, helping identify patients who may benefit from enhanced monitoring or modified surgical strategies. This simple, non-invasive CT-based tool supports individualized perioperative management in gastric cancer surgery. |
