Core Tip A recent study by Qi and Wu on CEA and CA19-9 as predictors of gastric cancer recurrence and chemotherapy toxicity is critically evaluated. Although the findings are clinically relevant, several methodological considerations, such as selection bias, temporal ambiguity in biomarker-outcome relationships, insufficient control of confounders, suboptimal survival analysis methods, and limited predictive performance assessment, restrict the clinical generalizability of the findings. There is a need for rigorous multivariable adjustment, proper time-to-event analysis, and validation of clinical utility before these biomarkers can be used to guide individualized postoperative management strategies.
