Duodenal lymphocytosis may pose a diagnostic challenge since it could be associated with different pathological conditions, which may or may not be related to gluten. In the present study, we demonstrated that during 2 years of follow-up of 85 patients with duodenal lymphocytosis, 27% developed celiac disease and 22% had a clinical diagnosis of gluten sensitivity; the remaining patients were affected by non-gluten-related conditions. At multivariate analysis, the haplotype DQ2/8 and the density of the intraepithelial CD3 lymphocyte infiltrate were the best predictors of the future development of gluten-related disorders. |
