Pancreaticoduodenectomy (PD) is responsible for significant morbidity and mortality. Pancreatic fistula (PF), main issue of this surgery, has been the subject of many release concerning its definition, its prevention and its management. However, some clinical presentations of severe PF after PD remain atypical and undescribed in the literature. We report an acute Budd-Chiari syndrome in relation with a compressive hematoma of the retrohepatic vena cava due to massive postpancreatectomy hemorrhage. Diagnosis was established by biological and radiological findings. An emergency relaparotomy was performed to remove the compressive hematoma, enabling rapid improvement. Awareness of this potential life-threatening complication may lead to avoid a delay diagnosis and to propose an appropriate therapeutic strategy. |
