Surgical formalin-fixed-paraffin-embedded samples are not representative of all pancreatic ductal adenocarcinoma (PDAC) patients and it was proved how “pre-resection” fine-needle aspiration smears are a better deoxyribonucleic acid (DNA) source. endoscopic ultrasound (EUS) is therefore the recommended method to obtain tumour’s molecular signature analysis. However, important limitations of EUS-acquired samples are: intratumoral heterogeneity, tumoral cells total amount and lesional-to-non-lesional cell ratio. Furthermore, sample handling and stock change DNA and even more ribonucleic acid extraction efficiency, a current “hot topic” as also patient-derived xenografts or organoids generation. Optimization and standardization of procedures from EUS-guided biopsy to samples handling and molecular analyses are, therefore, essential to allow a “precision medicine” for PDAC. |
