This review focuses on inflammatory markers recently emerged as indicators of tumor biological behavior and on immune state of patients submitted to liver transplantation for hepatocellular carcinoma (HCC), with a particular reference to the role of neutrophil-to-lymphocyte ratio. The impact of post-transplant immunosuppression on HCC recurrence is also analyzed according to the most relevant evidences published so far, which outline the importance of minimization of the use of calcineurin inhibitors and the protective role of inhibitors of the mammalian target of rapamycin.
