Core Tip Although the mortality rate in acute cholangitis has decreased notably due to early drainage and antibiotic use, it remains higher in severe cholangitis. Early identification of severe cholangitis can reduce the mortality of patients with severe cholangitis. Presepsin is a promising biomarker for the early diagnosis and prognosis of bacterial infections. In this study, presepsin was found to be superior to white blood cells, C-reactive protein, procalcitonin, and total bilirubin in predicting positive blood culture and assessing risk in patients with acute cholangitis in the emergency department.
