Core Tip The present case suggests that systemic thrombolysis should be safe and effective for acute extensive portal venous system thrombosis (PVST), if anticoagulation is unresponsive. Anticoagulation is the first-line choice of treatment for acute PVST. By comparison, systemic thrombolysis has been rarely attempted for the treatment of acute PVST due to its potential bleeding risk. The present case suggests that systemic thrombolysis should be safe and effective for acute extensive PVST if anticoagulant therapy is unresponsive.
