This is the largest study of anal fistulas reported to date. One thousand three hundred fifty-one fistula procedures were performed in 1250 patients over 14 years at an exclusive fistula-care center. An exact algorithm was followed- fistulotomy was done for simple fistulas, and a novel sphincter-sparing procedure, transanal opening of intersphincteric space, was performed for high complex fistulas. It resulted in an overall success rate of 93.5% in all fistulas (98.6% in simple fistulas and 86% in high complex fistulas. Fistulas associated with abscesses were managed safely and successfully by definitive fistula surgery in the first attempt. Several novel concepts were developed during the study. |
