A comprehensive diagnostic assessment is a critical first step in treating patients with mixed anxiety and depression. Practitioners should be alert to the possibility that this may be a concealed bipolar disorder since misdiagnoses rates can be 70%. Treatment in the patient with uncomplicated non-substance-abusing unipolar disorder may be quite straight forward. However patients are in all likelihood treatment-resistant with probable bipolarity and substance abuse. In the latter instance, more complex regimen of medications with combinations of pharmacotherapy are required. Finally, a “stacking” approach, to cover the full spectrum of available receptors targeted by current pharmacotherapy regimens is recommended. |
