The diagnosis of intrathyroidal papillary microcarcinomas has increased exponentially, but mortality remained stable. Although surgical resection was classically the standard therapy, active surveillance has emerged as an alternative approach aimed to recognize the minority group of patients who will develop clinical progression and would probably benefit from a rescue surgery. During the follow-up these tumors evolve with no changes in size, very slow growth and even decrease in diameter, including tumors between one and two centimeters. Given the high prevalence of papillary microcarcinomas and the excellent outcomes of observation, active surveillance constitutes a safe and feasible alternative in properly selected patients. |
