Core Tip The insufficient height of residual bone in the posterior maxilla is commonly encountered after tooth loss in elderly people. Commonly, when the vertical bone height is less than 5mm, the lateral antrostomy approach was recommended. In this case, cone-beam computer tomography examination showed an available vertical bone height in the bilateral posterior maxilla was 0.5-6.8mm in the left, 2.8-6.5mm in the right. We choose the crest approach with platelet-rich fibrin and bovine bone graft material and got a good prognosis. Through this case, minimally invasive implantation and repair of elderly patients were achieved, which reduced surgical trauma, postoperative pain, swelling, reduced surgical costs and significantly shortened the course of implantation and repair. Therefore, for the elderly patients, on the premise of strict mastery of surgical indications and operating skills, the minimally invasive method with simultaneous implantation in the bottom of the maxillary sinus can be considered.
