Core Tip The aim of this review is to provide a critical overview on neurological and psychiatric disorders of patients with hepatitis C virus infection, the underlying pathogenetic mechanisms and the effect of new direct-acting antiviral on extrahepatic symptoms. In this review are also reported the main clinical and pathogenetic differences with hepatic encephalopathy and the different diagnostic strategies in these two conditions.
