Core Tip This manuscript provides a different viewpoint to the controversial subject of off label placement of single piece acrylic intraocular lens (IOL) in contrast to case reports and case series from tertiary care referral centers. The retrospective series of cases illustrates that in select cases with sulcus placed single piece IOL observation may be a valid alternative management to IOL exchange.
